Build PACT_BR_PATIENT table with its logic from mapping specification in SQL

Requirement: 

Create a SQL script for the attached “mapping_specification_sheet” excel sheet and read the target table “PACT_BR_PATIENT“ and write it as “PACT_BR_PATIENT_sql“ with its logic.

Display the results
